General Internal Medicine: Refer Your Patients to the Virtual Ward

The Virtual Ward was developed as a post discharge model of care and was evaluated with a randomized controlled trial that was completed in 2013. We have since re-designed the Virtual Ward and now have a team including a GIM physician, an nurse practitioner, an OT, a pharmacist, a social worker as well as a CCAC care coordinator.

We aim to support complex patients who have been discharged from hospital or are deteriorating in the community and are at risk of ED visits or admissions. We can provide more intense care to patients over a number of weeks, including home visits.
