Trillium Health Partners is one of the largest community-based acute care facilities in Canada. Comprised of the Credit Valley Hospital, the Mississauga Hospital and the Queensway Health Centre, Trillium Health Partners serves the growing and diverse populations of Mississauga, West Toronto and surrounding communities and is a teaching hospital affiliated with the University of Toronto. Our Mission: A New Kind of Health Care for a Healthier Community When we set out to build our vision and future, we connected with our community - patients, families, visitors, physicians, staff and volunteers. The foundational goals of our new strategic plan - quality, access and sustainability - anchor everything we do. Our mission for a new kind of health care is built on an inter-connected system of care organized around patients - inside and outside the hospital. At THP, we are relentless in providing high quality, compassionate care to our communities and take great pride in fostering an inclusive and accessible environment and we are all accountable for contributing to a healthy, safe and respectful environment for healing and promoting excellence in patient care though advancing patient and staff safety. If you are passionate about what you do, motivated to improve the health of the community, committed to excellence, quality and patient safety we would like you to join our Better Together team! As partners in creating a new kind of health care, we are Better Together. The Trillium Health Partners Stroke Fellowship has been designed to provide the trainee with the opportunity to acquire the knowledge and skills necessary to support the diagnosis and evidence-based management of a wide range of cerebrovascular diseases. Upon completion of the fellowship, the trainee will develop the required knowledge, skills, professionalism, and education to ensure the best possible delivery of stroke care not only within our provincial network but on a national and international level. The primary goal will be for the trainee to be capable of independently managing any type of clinical situation related to all types of strokes and provide both hospital care and post-acute care with a focus on preventive community care. The fellow will work as a part of the stroke service team and be given an opportunity to play a significant role as a stroke lead in his/her community. The secondary goal will be for the fellow to gain an understanding of the Ontario Stroke Strategy, including the organization of provincial networks and knowledge of Quality Based Procedures for Stroke (acute and post-acute). As part of the fellowship the trainee will be given an opportunity to participate in quality improvement and research initiatives with the option to lead his/her own clinical research project (focusing on data registry and stroke trials). Under the supervision of a staff neurologist (experienced in stroke care) the fellow can select one of several topics as an area of focus, including but not limited to epidemiology, neuroimaging, meta-analysis, neuro-sonology and or economic analysis studies. At the conclusion of the fellowship, it is expected that the trainee will be able to sit for the stroke fellowship exam offered by the Canadian Stroke Consortium.
