General Description UHN/SHS Hospital Medicine Clinical Fellowship

 The UHN/SHS Clinical Fellowship in Hospital Medicine is suitable for physicians who have completed training in Family Medicine or Internal Medicine, and who wish to acquire additional expertise and experience in managing complex medical inpatients.  Fellows will gain competencies in areas such as management of common acute medical conditions, bedside procedural skills, health system navigation, quality improvement and patient safety, communication skills and teamwork. In addition to a focus on complex medical inpatients, our program offers the unique opportunity to work with specialized patient populations such as cancer, perioperative and psychiatry inpatients. This allows our graduates to meet the demands of the growing roles of Canadian hospitalists in co-managing hospitalized patients.

Our Program offers a 1-year fellowship, based at the Toronto General, Toronto Western and Mount Sinai Hospitals. Under exceptional circumstances and at the discretion of the Fellowship Trainee Committee, contract extensions up to an additional year can be offered. There are 16 available positions per year. Fellowships start on July 1, with some flexibility depending on the availability of vacant positions. This position is suitable for residents interested in doing full-time or part-time hospitalist work, rural and/or suburban locums managing acute medical inpatients and those who are looking for further enhanced skills in Hospital Medicine. The interface working with specific patient populations (SHS – Orthopaedics co-management, UHN – Oncology) allows for graduates of our program to leave with unique skill sets that confer greater marketability.

Clinical Fellows rotate through each of the Hospital Medicine Service sites, working closely with the interprofessional team and supervising faculty. As Clinical Fellows gain more knowledge and experience, they are also given increasing responsibility. In addition to working weekdays, Fellows are responsible for in-house call whereby they are responsible for inpatient coverage and ER admissions with 4-5 calls (2-3 weekday nights and 2 weekend 24-hour coverage shifts). Clinical training is centered on daily patient care activities, including reviewing all new admissions, daily discussions around the ongoing management of patients on the service, and comprehensive discharge planning involving all trainees, supervising faculty, and the allied healthcare professionals on the team.

The Hospital Medicine team is supervised by a faculty member from the Division of General Internal Medicine.  The team includes Hospital Medicine Clinical Fellows, elective residents in Internal Medicine and Family Medicine residents, medical students, and various inter-professional team members. As a Clinical Fellow, you will develop competencies in emergency department consultations, inpatient management of common acute medical conditions, perioperative management, discharge and transitions planning, teamwork, quality improvement, and bedside procedural skills.
