Adult Endocrinology and Metabolism CBD FAQ July 9, 2025
Our program launched the Royal College’s Competence-Based Design (CBD) curriculum on July 1, 2025.
Below are some common questions that have been received from faculty and residents.
1. What are the major differences between CBD and the current curriculum?
CBD is competency based, not time-based. There are four stages in residency (Transition to Discipline (TTD), Foundations of Discipline (FOD), Core of Discipline (COD) and Transition to Practice (TTP). The Competence Committee will assess each resident’s performance, to decide when residents are ready for the next stage. This decision is based on their completed Entrustable Professional Activity (EPAs), ITARs (In-Training Assessment Report), written examinations, patient assessments, self-reflections, along with other formative assessments.
2. Which residents will be in CBD?
The new PGY4s will be in CBD for both years of their training, and will be assessed with EPAs, ITARs.
The new PGY5s will remain as the final cohort assessed with MIniCEXs, ITERs. Both EPAs and Mini-CEXs are on the Elentra platform. ITARs and ITERs are on POWER, accessed only by the supervisor for the rotation.
3. What does the launch of CBD in Endocrine mean to me as a faculty member?
You may be asked to complete EPAs for the PGY4s.You will be asked by the rotation supervisor for feedback for the residents’ ITARs, if you have worked with them on the rotation. You should be familiar with the Rotation Plan for the rotation that they work with you on. This outlines the EPAs assigned to the rotation, the objectives (which are assessed in the ITAR) and further details about expectations of the rotation. (see below for more information).
4. When should I complete an endocrine EPA?
When assigned one for a clinic by the rotation coordinator, if a resident asks for one and/or if you feel an encounter warrants an EPA for documentation.
5. How can I do a great job completing an EPA?
Our program’s goal is to provide meaningful feedback to learners. When completing an EPA, please:
· Ideally, directly observe some component of the patient interaction
· Complete the form together, on the same day as the encounter
· Decide with the resident which EPA will be done that day (this will be TTD1 for the first block: “Assessing patients with a known Endocrine condition”)
· Choose the encounter to assess & complete the EPA on the Elentra platform
We are de-emphasizing the importance of the overall scale as part of a pilot study seeking to improve the quality of feedback.
Helpful, detailed comments are mandatory – both strengths and what can be worked on “to get to the next level.
6. How can I put Elentra (the EPA website) on my phone?
7. What do I need to know about Rotation Plans and ITARs?
We have attached the following documents to this email, so you can familiarize yourself with them:
· The Rotation plan for the Core Hospital Rotation
· The Stage Primer for Transition to Residency, which takes place in months 1-2 of the academic year
· The Primer for TTD 1 EPA (the only EPA for this stage)
· The Curriculum Plan which summarizes which EPAs are completed in the TTD Stage
8. Where can I find out more about CBD in endocrinology?
Please visit the CBD folder on the new Endocrine program Sharepoint platform, accessible on the Endocrine section of the Department of Medicine website.
